Basic Qualifications
-
Graduation from an approved program in Social Work
-
Valid CPR-card

Responsibilities
-
Document history, care plans & prepare reports as needed
-
Evaluate patients’ family/social situation to support a treatment plan
-
Evaluate patients’ psycho-social needs
-
Make recommendations to the healthcare team for appropriate psycho-social support
-
Participate in on-going counseling
-
Utilize community resources to assist patients’ psycho-social needs
