Basic Qualifications

  • Graduation from an approved program in Social Work

  • Valid CPR-card

Responsibilities

  • Document history, care plans & prepare reports as needed

  • Evaluate patients’ family/social situation to support a treatment plan

  • Evaluate patients’ psycho-social needs

  • Make recommendations to the healthcare team for appropriate psycho-social support

  • Participate in on-going counseling

  • Utilize community resources to assist patients’ psycho-social needs

  • Communicate with people outside the organization

  • Deal calmly & effectively in high-stress situations

  • Develop constructive & cooperative working relationships with others & maintain them over time

  • Observe, receive, & otherwise obtain information from all relevant sources

  • Represent the organization to patients, clients & the public

  • Skills in critical thinking, problem-solving, logic & reasoning, judgment, time-management techniques, & data entry
